Document an Event

Documenting Events is critical to show the actions you took to prevent an adverse event from occurring.  Or to record the details that lead to the occurrence of an adverse event.

Introduction

While monitoring each patient and verbally redirecting them to maintain patient safety is your highest priority, documenting Events is a key responsibility.  It’s critical for the care team to have a complete and detailed record of material events throughout the patient's care history.  

It is important to be clear and factual in your descriptions without placing blame or making assumptions. The Notes topic can be used when you feel it appropriate to record an event that does not rise to the level of an adverse event.

Creating an Event

Best Practice

A typical non-sleeping virtually monitored patient will normally have 1-5 documented Events per shift.  Very active patients that require more interventions may require more.

As a guideline, you should document anything you would normally note as a 1:1 sitter.

Events can be created by hovering over a Live Video window and clicking the "Event" button.

Event Module Details

Event Types

  • Adverse Event - Prevented: your actions and the alarm prevented the outcome (ie. you were able to keep them in bed and a nurse arrived before the patient fell)

  • Adverse Event - Occurred: in spite of your best actions, the adverse event still happened (ie. the patient fell)

  • Note: other situations - typically simple things like - “Pt. left the room for physical therapy.”, “Pt. returned to room”, “Informed PCA patient wanted to use the bathroom”

Time of Event

Defaults to current date/time but can be edited

Category

  • Assisted fall
  • Unassisted fall
  • Elopement
  • Line, tube or drain dislodgement
  • Verbal Abuse
  • Physical Abuse
  • Seizure Activity
  • Intentional Self Harm
  • Suicide

Edit an Event

You are the Original Author of the Event

If you are the author of the Event entry, you can edit it - mistakes happen. You can change the Event Type, Category, and Description but not the Time.

Simply hover over the Event entry and click the "pencil" icon in the top right corner. In this example, the Category probably should have been Elopement as opposed to Other.

Hover over Event to edit

This will open the Event window for any necessary changes.

When your changes are complete, hit the Submit button. You can see the history of edits/changes by clicking on the Event entry again.

You are not the Original Author of the Event

Sometimes it is necessary for a Manager to correct an entry from a user who might not be available to make their own corrections. If the user / manager has the proper access, they can make the edit for the user.

Users with the “Manage Video Monitoring > Edit any patient monitoring event” privilege, have the ability to edit Alarm and Event entries for all other users. All edits will be noted by the date/time of the edit and the person making the edit.

Patients with Multiple Monitoring Sessions

If the patient has more than one monitoring session (possibly from a previous stay), all events will be displayed for the continuity of the patient's care. The old event entries will be displayed following a section break.

Expand Mode

This enables easier review and visibility if many more events / notes / alarms per screen before scrolling is required.
