Form Builder

Allows administrators to create checklists and forms available to users to capture structured information (e.g. a Mobility Assessment, Delirium Assessment, Braden Scale - Pressure Sore Risk, Virtual Pain Assessment, etc) about monitored patients in just a few minutes. Once forms are created and made active, (authorized) users can use them on monitored patients. Users can then view the results of those forms on the patient detail pages.

Creating Forms

To create a new form, click the "+ Add Form" button. Complete the required fields.

• Name: name of the form
• Description: optional description field
• Active: toggle yes/no

Forms can be created with any combination of the below with simple drag and drop controls.  You can also preview your form by clicking the Preview tab.

Editing Forms

Forms can be edited by clicking the Edit button in the Form List page.
